APPOINTMENTS
To schedule and appointment at any office, you may call the main office numbers: (937)294-1489 or 1 (888)824-1489. We have a live operator!

If you are referred to one of our doctors, you will be given his next available appointment. You may choose an appointment with any of our physicians who have a closer or more convenient date to meet your needs.

Before your first visit, we will mail some forms for you to complete at home. We want to know about the reason for the visit, your present and past medical conditions, allergies, etc. A form requesting your insurance information will also be included.

EMERGENCIES
If you feel you need to be seen as an emergency, we would like for your referring physician to contact us and give us a brief history of your problems. If a medical emergency arises when our office is closed, one of our physicians is always available to take your call. Just call the Montgomery County Medical Society at 463-1633 and they will put you in touch with the doctor on duty.
PRESCRIPTION REFILLS 
Please check your monthly medication supply before each appointment so you can arrange for refills while you're at the office. If you run out of a routine medication between visits, call our office at (937)294-1489 and ask for your physician's prescription refill mailbox. Please leave your:

Please try to call us at least 48 hours before your medication runs completely out so you can be sure your prescription will processed before you miss a dose.

Emergency prescriptions - To ensure your prescription is ready for pickup at your pharmacy, please call ahead.

Weekends - It is the policy of our office not to refill pain medications during weekends. If you forsee the need to refill a prescription, call on Friday before 3 p.m.

HOSPITAL AFFILIATIONS
Our physicians are on staff at Kettering Memorial Hospital and Sycamore Hospital.

INSURANCE ACCEPTED
Most of the insurances available in the community are welcome in our office. When calling for an appointment, please specify the name of your plan and make sure it is accepted by our group. We accept most major health insurance plans including Medicare.

PHYSICIAN REFERRALS
If you've been referred to our practice by your family physician, we will keep in touch with him/her during your treatment. Because we are a specialty practice, many insurances require a written referral from your family doctor. Make sure that the appointment forms of referral from your family physician have been forwarded to our office, otherwise, you will be responsible for the charges originated from that visit or your appointment will need to be rescheduled.

FEES AND BILLING
We submit all insurance claims for our patients. Your insurance may carry some form of co-payment. Be prepared to make your co-payment at the time of your visit. We accept checks and major credit cards. Patients without insurance are required to pay at the time of service.
